End-User License Agreement (EULA)

This EULA is between you ("Licensee," a medical practice or provider) and VeriPhy Health, LLC ("Licensor"). By installing or using VeriPhy Health ("the Software"), you agree to these terms.

1. License grant

Licensor grants Licensee a non-exclusive, non-transferable license to install and use the Software on computers owned or controlled by a single practice location (or the number of locations stated on your invoice), solely to verify insurance eligibility for that practice's own patients.

The Software is licensed, not sold. Licensor retains all intellectual-property rights. Any verification results the Software produces belong to you.

4. Updates & support

Payer portals change over time. Updates and support are provided only under an active Care or Updates plan, if purchased. Without such a plan, the Software is provided as-is at the version you received.

5. Term & termination

This license continues until terminated. It terminates automatically if you breach it. On termination you must stop using and delete the Software.

6. No warranty; limitation of liability

THE SOFTWARE IS PROVIDED "AS IS," WITHOUT WARRANTY OF ANY KIND. Licensor does not warrant that eligibility results are accurate, complete, or current — payer data can be wrong, delayed, or change by date of service. TO THE MAXIMUM EXTENT PERMITTED BY LAW, LICENSOR'S TOTAL LIABILITY IS LIMITED TO THE AMOUNT YOU PAID FOR THE SOFTWARE IN THE 12 MONTHS BEFORE THE CLAIM, AND LICENSOR IS NOT LIABLE FOR DENIED CLAIMS, LOST REVENUE, OR OTHER INDIRECT OR CONSEQUENTIAL DAMAGES. You remain responsible for independently confirming benefits before relying on them.

By using this website or purchasing VeriPhy Health you agree to these terms.

Privacy Policy

Patient data (PHI)

VeriPhy Health runs entirely on your own computer. It does not transmit patient information to VeriPhy Health or any VeriPhy Health server. Verification results are saved only into a folder on your machine. Your patients' data goes only to the payer portals you sign into yourself — the same portals your staff already uses. Because VeriPhy Health does not receive or store PHI through the Software, it does not act as your Business Associate for use of the Software; you remain the custodian of your patients' data.

Website & account data

When you contact us or buy a license we collect what you provide (name, practice, email, phone) and standard payment details processed by our payment provider (Stripe); we do not store full card numbers. We use this only to sell and support VeriPhy Health. We do not sell your information.

Support sessions

If you request support, do not send us patient information. Share only screenshots with patient details removed.

It does not guarantee coverage, payment, or accuracy, and it does not replace your obligation to confirm benefits with the payer before providing or billing services. It does not process prior authorizations and does not determine whether your provider is in a patient's network.
